Your First Visit

Welcome New Patients!

We always welcome new patients and try to offer convenient appointment times.

Please contact our office to schedule an appointment or use our Appointment Request Form.

Your first visit should last approximately one hour. In case skin testing is necessary, please do not use antihistamines for 3-5 full days before your visit, if possible. Please do not stop any other medicines. A list of common antihistamines appears below for your convenience. Fasting before your visit is not necessary.

Please bring all of your current and recent medicines or a list of these medicines as it is important for us to know what you are on now and what was used in the past.

If skin testing is performed, please keep in mind that you will have to hold still for a few minutes and a babysitter may be necessary if you have young children.

Please bring with you any recent blood test results, chest X-ray, chest and sinus CT scan reports if appropriate. The actual X-ray films are not necessary.

The office staff will ask for your insurance card and referral form if your insurance plan requires one. Payment of your co-pay is expected at the time of your visit. Please note that the office accepts cash, checks, credit and debit cards.

Lastly, please bring in your primary care doctor's full name and address so that a summary letter can be sent to your doctor upon completion of your evaluation.
